You will educate patients and their family members and ensures the safety of the patient. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Develop an individualized ST plan of care and makes adjustments as needs change. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function. Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decision. Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy. Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.Operations: Attend in-service training and mandatory agency meetings.HARD & SOFT SKILLS:Compassionate communicator with a positive attitude.Patience is a virtue when working with patients, families, physicians, and coworkers.Attention to detail is critical, as is being observant and following directions. REQUIREMENTS:Speech Therapist with a current license in the state of employment. Minimum one-year experience as a ST in an acute care setting. Home care experience preferred. Valid driver's license and auto insurance in your name as a driver.
